A statutory demand for the payment of a debt owed by a company must be made in form 509H and signed by the creditor or the creditor's solicitor.

If the debt is not a judgment debt, a statutory demand must also be accompanied by an affidavit which verifies the debt.

The demand must then be properly served upon the company.
